Aged Care Support

At Living Bridge Community Care, our care management services are designed to help you live independently and comfortably, ensuring you have

the support you need to thrive in your community. Below are some of the services available through your Home Care Package.

Domestic Assistance

Care Workers can assist with various household tasks, acting as an extra pair of hands to help maintain a clean and comfortable home. Services include:

    • Cleaning
    • Vacuuming
    • Mopping
    • Laundry
    • Changing bed linen and remaking the bed
    • Meal preparation
    • Washing dishes
    • Taking out the rubbish

Domestic Assistance is not the same as a professional cleaning services and is tailored to each individual, providing personalised help where it’s needed most.

Social Supports and Community Access

Care Workers help you stay connected and active within your community. Services include:

  • Shopping assistance
  • Medical and health appointments
  • Paying bills
  • Collecting the mail
  • Attending functions
  • Friendship outings
  • Hairdresser and beauty appointments
  • In home social support for companionship
Light Gardening

Maintain your outdoor spaces with light gardening services, which include:

  • Lawn mowing
  • Whipper snipping and light pruning
  • Weeding 
  • Clearing leaves
Meal Preparation

Meal preparation and delivery can be included as a service within a Home Care Package 

 

  • Receive assistance with meal planning, grocery shopping, cooking, and meal delivery, which can be especially helpful for those with mobility or health challenges. Funding from the HCP typically covers a portion of the meal costs, with the participant contributing a co-payment for ingredients
Transportation

Transportation services help you maintain your independence and access the community with ease.

Personal Care

Personal Care specifically focuses on helping individuals with their personal hygiene and grooming needs and are designed to support individuals who may have difficulty performing these tasks independently. Personal Care services are delivered by Certified Care Workers and can include:

  • Bathing and showering assistance
  • Dressing and grooming assistance
  • Brushing teeth
  • Toileting
  • Skin and hair care
    Clinical Care

    Clinical Care is delivered by

    • a registered nurse
    • an enrolled nurse
    • a registered allied health professional, such as an occupational therapist, physiotherapist, exercise physiologist, remedial massage therapist, speech pathologist and podiatrist.
    End of Life Pathway

    Many older people wish to remain in their own homes with their family as they near the end of their life. 

    The End-of-Life Pathway is the government’s new program through the Support at Home program commencing 1st November 2025. 

    The Support at Home program is Australia’s new in-home care system. It will replace the Home Care Package program. 

    The End-of-Life Pathway provides additional funding and support for individuals with a terminal illness expected to live less than three months, enabling them to remain at home for end-of-life care. This pathway offers up to $25,000 in funding over 12 weeks, and if needed, services can continue for an additional 4 weeks (16 weeks total). 

    Accessing the End-of-Life Pathway: 

    Existing Support at Home participants can access the End-of-Life Pathway through a high-priority Support Plan Review conducted by an aged care assessor. This ensures they don’t need to undergo a full reassessment at an already difficult time  

    People not currently receiving Support at Home services will be referred for a high-priority aged care assessment. 

    The End-of-Life Pathway covers a range of essential services: 

    • Nursing Care 
    • Personal Care (e.g. showering, toileting) 
    • Domestic Assistance 
    • Social Support 
    • Meal preparation and delivery 
    • Transport to and from appointments 
    • Assistive Technology (if clinically prescribed) 
    • Allied Health Support. Delivered by a registered allied health professional, such a social worker, occupational therapist, physiotherapist, exercise physiologist, remedial massage therapist, speech pathologist and podiatrist.

    Navigating end-of-life care can be an emotional and overwhelming process. Living Bridge Community Care believes every person’s final journey should feel truly theirs with comfort, compassion, and dignity. From 1st November 2025, Living Bridge Community Care will be offering the new End-of-Life Pathway within the Support at Home program.
